Philip Michael Dvorak, 59, of Columbus died Monday, July 17, 2017 at Gentiva Hospice.
A Funeral Mass will be held at 1:00 PM Friday, July 21, 2017 at St. Anne's Catholic Church with Father Brian O'Shaughnessy officiating. Visitation will be held after the services.
Mr. Dvorak was born October 30, 1957 in Fort Rucker, AL the son of Nora Dvorak of Iowa and the late Philip Joseph Dvorak. He was a Logistic Financial Advisor with Valley Hotsy.
Other than his mother Mr. Dvorak is survived by his wife, Cherie Dvorak of Columbus, daughters, Lauren Dvorak of Columbus and Katharine Johnson of Oklahoma, son, Michael Dvorak of Arizona, sisters, Virginia Dvorak of California, Laura Cullen of Iowa, Ann Tuhl of Colorado, brothers, Bill Dvorak of Florida and Mark Dvorak of Minnesota along with several nieces, nephews, aunts and uncles.
